Tell us about your organization. Once we approve your application, we email you a facility code your Game Masters and players use to sign up.

A ZQuad reviewer reads it and either approves it or replies with a reason.
On approval, you get an email with your facility code.
You share that code with your Game Masters and players so they can join your facility.

This form sends your application, and any certificates you add, straight to our applications team. Your files are stored privately and only our team can open them.
